Conversione del titolo per PC Engine con gli sprite dei personaggi modificati per farli somigliare ai protagonisti della serie di anime da cui il gioco prende il titolo.
Dopo aver salvato la sua fidanzata dalle grinfie di un malvagio stregone nel precedente gioco, Tom Tom, ossia Wonder Boy, in questo gioco conosciuto anche col nome Shien, giunge a Monster Land (quasi) completamente nudo. All'inizio del gioco Tom Tom incontra un indovino (Fortune Teller): in realtà è un cinghiale, ma in Monsterland gli animali hanno i loro negozi, per esempio il barista è un dinosauro o una volpe, il venditore di armature un maiale. L'indovino spiega al protagonista che il loro mondo è minacciato da un terribile drago. Tom Tom è un uomo (o ragazzo) coraggioso, e l'indovino regala a questi una vecchia spada (Gradius) e una pozione magica (Revival Pill) che consente di riacquistare tutta la vitalità invece di morire. Dopodiché Tom Tom inizia il suo viaggio in Monster Land. A differenza del primo Wonder Boy, dove lo scopo del gioco era salvare la fidanzata dalle grinfie di un perfido stregone, qua la missione sembra essere quella salvare l'umanità (o almeno il Monster Land). Non viene fatto alcun riferimento alla fidanzata di Tom Tom, e il fatto che il protagonista sia conosciuto anche con un altro nome (Shien), lo stesso gameplay (da platform a RPG) e l'ambientazione del gioco siano "stravolte" (da un mondo preistorico a un mondo fantasy medioevale) farebbe pensare che i due titoli non siano parte della stessa saga.
